IMPORTANT: Please read the following instructions BEFORE you begin:
- Please be aware that once you begin completing the profile you must finish in one sitting. It is not possible to stop and then come back to finish later.
- Please allow yourself 20-40 minutes to complete the profile. DO NOT LEAVE THE PROFILE UNATTENDED- your connection will time out and you will need to start over.
- This site DOES use a pop-up window when you are beginning the profile; please allow the pop-up window accordingly if prompted by your browser or pop-up blocker.
- Wireless Connection Users: Please note that wireless internet connections can be unstable and loose connectivity for short periods of time. For this reason, we suggest that you DO NOT access the profile utilizing a wireless internet connection.
- Mac Users: Safari browser is NOT supported. Please use Internet Explorer or Firefox.
- On the first screen you will be asked to enter your first and last name along with the last 4 digits of your social security number. This is the only information that is necessary for you to enter.
